Techniques used in a nursing assessment

Aside from the taking of a person’s vital signs, techniques that are used in a nursing assessment are inspection, auscultation, palpation and percussion.  In undertaking an assessment process, the patient should be asked pertinent questions regarding his condition or his perception of his condition.  Sometimes beliefs play an important part in a person’s feeling of personal well-being.

The nursing assessment is recorded in the medical records of the patient.  Files may be kept on filing cabinets but the modern way of keeping records is through electronic means which can be easily accessed by any team member.
